The 5 Whys
The 5 Whys is a problem-solving technique used to explore the root cause of an issue by asking "why" repeatedly until the underlying cause is identified. This method is particularly effective for identifying the root cause of a problem, rather than just addressing the symptoms.
How the 5 Whys Work:
- State the Problem: Clearly define the problem you're experiencing.
- Ask the First Why: Why did the problem occur? Identify the immediate cause of the issue.
- Ask the Second Why: Dig deeper by asking why the first cause happened.
- Ask Subsequent Whys: Continue this process, typically around five times, until you reach the root cause. Each answer forms the basis of the next "why" question.
- Identify the Root Cause: The goal is to reach a fundamental cause that can be addressed to prevent the problem from recurring.
Example:
- Problem: The car won't start.
- Why? The battery is dead.
- Why? The alternator isn't working.
- Why? The alternator belt is broken.
- Why? The belt was not replaced on time.
- Why? The car maintenance schedule was not followed.
